New Floor Stock Order Form at Guthrie Corning Hospital

Starting on Tuesday, Nov. 6 at 8:30 a.m., the existing Lotus Notes Stock Refill Database used at Guthrie Corning Hospital will no longer be usable. This database will be replaced by a new SharePoint application. This application can be accessed via the intranet or by clicking here.

This link will provide you with a form to complete in order to access inventory items. Once you complete this form, click the submit button at the bottom of the page to place your order. You will receive a confirmation message when the order has been successfully placed.
